Heteropaternal Superfecundation: Brazilian Woman Gives Birth to Twins with Two Different Biological Fathers

A 19-year-old woman in Brazil discovered that her twin boys had two different biological fathers after DNA testing revealed an extraordinarily rare phenomenon known as heteropaternal superfecundation.

The case, reported in Mineiros, in the Brazilian state of Goiás, occurred after the young woman had sexual relations with two different men on the same day. She later became pregnant and gave birth to twins. Initially, she believed that both children had the same father.

When the twins were about eight months old, however, she decided to carry out a paternity test with one of the men. The DNA examination confirmed that he was the biological father of one of the boys but excluded him as the father of the other.

The unexpected result led the mother to remember that she had also been with another man around the same period. A second DNA test was subsequently carried out, confirming that the other man was the biological father of the second twin.

The unusual situation is medically known as heteropaternal superfecundation, a rare reproductive event in which two eggs released by the same woman during the same ovulatory period are fertilized by sperm from two different men.

Although it may sound almost impossible, the biology behind it is straightforward. A woman can occasionally release two eggs during one ovulatory period. If she has sexual intercourse with different men during her fertile window, sperm from each man can potentially fertilize a separate egg. The two fertilized eggs can then implant in the uterus and develop simultaneously as fraternal twins.

The children therefore share the same biological mother but have different biological fathers. Genetically, they are paternal half-siblings who happened to be conceived during the same reproductive cycle and carried together in the same pregnancy.

The phenomenon is extremely rare and can easily go unnoticed. Unless there is a reason to question paternity or DNA testing is performed, there may be nothing to indicate that twins have different fathers. Their physical appearance alone cannot reliably establish their biological parentage.

In the Brazilian case, DNA testing provided the decisive evidence. The two boys were twins in terms of their pregnancy and birth, but their genetic paternal lineages were different.

The case was reported in Brazilian media in 2022, and physician Túlio Jorge Franco said at the time that it was approximately the 20th such case reported worldwide. That figure referred to documented cases known at the time and should not be interpreted as a definitive estimate of how often the phenomenon occurs.

Medical literature has also documented other cases of heteropaternal superfecundation. Researchers note that the true frequency is difficult to establish because many cases would never be detected, particularly where paternity is not disputed and genetic testing is not undertaken.

Heteropaternal superfecundation should not be confused with superfetation, another exceptionally unusual reproductive phenomenon. Superfetation involves conception occurring after a pregnancy has already begun, whereas heteropaternal superfecundation involves the fertilization of separate eggs during the same ovulatory period.

The Brazilian case is a striking reminder that human reproduction can produce genetic circumstances that are invisible without scientific testing. Two children can share the same mother, develop together in the womb and be born as twins, yet have different biological fathers. What appeared to be an ordinary twin pregnancy therefore became an extraordinary genetic discovery: one mother, two twins and two different biological fathers.
